Community Care Hubs of Washington

Connecting Washingtonians to services and resources 

Health isn’t just about what happens at the doctor’s office or clinic. It also depends on peoples’ everyday needs being met. In 2025, the ACHs began offering care coordination services through the nine regional Community Care Hubs of Washington.

Supporting a community-based network 

Community Care Hubs of Washington amplify and coordinate the great work of established local organizations. We partner with and invest in 141+ network partners. We connect these trusted, local organizations with administrative coordination, funding, training and tools so they can offer culturally appropriate, local, one-on-one support to a community-based workforce.

Better Health Together (BHT)

Counties covered: Adams, Ferry, Lincoln, Pend Oreille, Spokane, Stevens. Tribe(s) with reservations within ACH geographical boundary: Spokane Tribe of Indians, Confederated Tribes of the Colville Reservation, Kalispel Tribe of Indians.   

GREATER HEALTH NOW (GHN)

Counties covered: Asotin, Benton, Columbia, Franklin, Garfield, Kittitas, Walla Walla, Whitman, Yakima. Tribe(s) with reservations within ACH geographical boundary: Confederated Tribes and Bands of the Yakama Nation.

Olympic Community of Health (OCH)

Counties covered: Clallam, Jefferson, Kitsap. Tribe(s) with reservations within ACH geographical boundary: Hoh Indian Tribe, Jamestown S’Klallam Tribe, Lower Elwha Klallam Tribe, Makah Indian Tribe, Port Gamble S’Klallam Tribe, Quileute Tribe, Suquamish Tribes.   

CHOICE

Counties covered: Cowlitz, Grays Harbor, Lewis, Mason, Pacific, Thurston, Wahkiakum. Note: CHOICE spans two purchasing regions.
